1. Owner / agent of dog from which semen will be drawn:

2. Stud sire from which semen will be collected:

Whereas the person mentioned in 1 above is the owner of the stud dog mentioned in 2 above.

And whereas Trade Genetics Frozen Semen Centre utilises the pelleted semen technology of Camelot Farms under agreement, and whereas the person mentioned in 1 above or their agent have presented the stud dog mentioned in 2 above, for semen collection, assessment and semen freezing, the parties agree as follows;

a) Trade Genetics Frozen Semen Centre will collect semen from the the stud dog mentioned in 2 above and preserve same. The cost of collecting, assessment and placing in storage will be paid for by the person mentioned in 1 above.

At a cost of €xxx.00 plus a storage fee of €50.00 per cane / vial per annum.

b) I, the person mentioned in 1 above, as the owner of the stud dog mentioned in 2 above, hereby grant permission for Trade Genetics to preserve semen of the above named dog.

c) Trade Genetics agrees to store the abover mentioned semen for a charge of €50.00 per cane / vial per annum which is payable annually twelve months in advance. If storage remains unpaid for more than thirty (30) days then Trade Genetics may take possession of the semen or destroy same after proper notification to registered owner of dog.

d) I, the person mentioned in 1 above, hereby agrees to notify of any change of address different to that detailed in 1 above at the time of such move to avoid storage payment default or unaccounted canes for those sires listed with Trade Genetics.

e) Trade Genetics agrees to produce and maintain all documents associated with freezing, storage and relocation of the semen and to provide copies, if required of documents required by any registration authority. Should any situation arise wherein Trade Genetics cannot maintain the storage of semen, the person mentioned in 1 above shall be notified thirty (30) days in advance.

f) The parties hereto hereby release each other and agree to hold each other harmless of any act of god or any circumstance including negligence, which may occur at any time in the collection, storage, preservation or insemination of the semen.
