CONSENT DECLARATION:

I give my consent to the Trade Genetic's representative to perform a surgery / surgical implant on the above named animal under general anaesthetic.

I understand that all anaesthetic techniques and surgical procedures involve some risk to the animal.

The animal will be discharged from the Trade Genetics veterinary surgery with dissolvable sutures and a post operative sheet detailing all aftercare instructions.
